Myeloperoxidase (MPO) is an iron-containing glycoprotein found in neutrophils and monocytes. It forms a tetrameric complex with a molecular weight of 150 kDa, composed of two glycosylated alpha chains (59-64 kDa) and two unglycosylated beta chains (14 kDa). Upon microbial invasion, MPO is released from neutrophil granules, converting hydrogen peroxide and chloride ions into a potent oxidizing agent called hypochlorous acid. MPO serves as a marker for airway inflammation in asthma and environmental irritant exposure. It’s also implicated in atherogenesis and a potential contributor to atherosclerosis. Elevated MPO levels in serum are linked to cardiovascular disease (CAD), making it valuable as an inflammatory marker to identify CAD patients at risk of cardiac events without myocardial necrosis.
